We are seeking a motivated and organised Care Coordinator to join our team.
The role involves, coordinating daily care rotas, communicating effectively with carers, clients and families, maintaining accurate care records and schedules, supporting care planning.
Liaising with professionals and external agencies when required.
Ensuring compliance with safeguarding standards and company policies.
The successful candidate will demonstrate integrity, kindness, and a genuine commitment to working with adults in the community.
Must have experience in a care setting with a good understanding of care practices and person centred care and safeguarding, alongside strong organisational and communication skills and good IT skills.
Applicants are required to have 2 satisfactory references and a full enhanced DBS check.
