Key Accountabilities:
Plan, coordinate, and implement the safe transition of new individuals into the service and community as needed.
Communicate with the families of the individuals we support and relevant members of multi-disciplinary teams, community groups, and voluntary bodies to advocate for their needs.
Plan, develop, and implement individual and group support programmes to enhance and maximise the capabilities and independence of individuals in the service.
Coordinate regular assessments and reviews using appropriate assessment models.
Address the physical, emotional, and personal needs of individuals, ensuring that work is focused on individual needs and upholds principles of respect, privacy, dignity, fulfilment, independence, and choice.
Ensure that medication is held, stored, and administered according to Praxis Care's Medication Policy and current statutory and legislative procedures.
Provide effective leadership, supervision, and management to Support Workers, including monitoring time and attendance and participating in capability, disciplinary, and grievance procedures as necessary.
Participate in organisational learning and development opportunities while identifying personal development opportunities for themselves and Support Workers.
Ensure that adequate records are created and maintained and that necessary reports are produced in compliance with organisational policy and statutory requirements.
Assist the manager with compliance concerning all relevant regulatory bodies.
Requirements of the Role:
Participate in the company appraisal and staff review process.

Engage in formal and informal supervision as required.
Carry out all duties in accordance with Praxis expectations, regulations, and policies.
Be prepared to take on additional duties as necessary.
Work the necessary hours to meet the needs of the service.

Essential and Desirable Criteria:
Essential:
2 years' experience in a health and social care setting and currently enrolled in QCF Level 3 in Health and Social Care, or understanding of the mental health/learning disability area.
QCF/RCF Level 3 in Health and Social Care.
Social Work.
First Level Nursing.
Allied Health Profession.
Level 5 Diploma in Leadership for Health and Social Care (adults management/adults residential management, Wales & NI).
Desirable:
Experience in mental health or learning disability services.
Full valid driving licence and access to own vehicle (If a disability precludes an individual from holding a driver's licence, it should be stated how they can meet the mobility requirement of the post).
